Please click on the EPFO and click the Download option on Form 10C and Form 19. Take the print out. Fill it up as per the instruction available in the same. and Submit the same to your ex office.

i have worked in the company fr 26.06.2013 to 31.12.2017, Last month (ie DEC 17) contribution not showing in the PF portal, thats why pf authority not accepting the PF withdrawal form.
can i ask them to refund/withdraw my PF amount Upto NOV 2017 & pending dec month amount after resolving issue. pl suggest
