Default Disciplinary action

Hi
I want to take suggestion from all of you that in my organisation one of our employee always absent duty without any intimation and after show cause he always submit medical certificate and accordingly case has been considered .Now a days he is habitual in this type of misconduct.Kindly suggest what type of action to be taken against such type emplyoee.
