Question ESIC Form 37

Hi
Everybody,

I have some confusion regarding Form 37 in ESIC. Pls clearify why it used and if possible pls send its format also.

Greetings

Form -37 is a form which shows the continution of employement where we need to declare employee is working for the concerned contributin period.

Please find the attachment for the same.
