INSTRUCTIONS FOR EMPLOYEE ENGAGEMENT SURVEY

 

The objective of the survey is to obtain employee opinion on various issues of concern related to the job and the organization. The questionnaire is divided into 4 sections:

 

SECTION A – Organizational Survey

This section consists of THIRTEEN broad parameters, and statements under each parameter. You are requested to encircle the number (1/2/3/4/5/6) which best describes your attitude towards a particular statement.

Rating Scale: A six-point rating scale is used, where

1 – Strongly Disagree                                                          2 – Disagree

3 – Somewhat Disagree                                                       4 – Somewhat Agree

5 – Agree                                                                               6 – Strongly Agree

For each statement, select the response that best represents your opinion. If you don’t have an opinion or knowledge about a certain question, or if it doesn’t apply to you, circle on ‘No Opinion’ option.
